You will be part of a supportive and established team dedicated to achieving positive outcomes for looked after children and care leavers. Key Responsibilities: - Manage a caseload of young people aged 16-18 within the Children in Care pathway. - Undertake statutory visits, reviews, and care planning in line with legislation and local procedures.
You will work closely with the Clinical Nurse Specialists and the Multi-Disciplinary Teams within the specialties as well as the rest of the Cancer Services Team. The Cancer Management team is committed to the personal development of team members and there will be opportunities for development within this role.
You will help to create a warm, stimulating environment whilst working a mix of day shifts and sleeps to meet individual needs. You'll do more than provide care – you'll be a mentor, advocate, and source of encouragement, helping the people you support to achieve their own individual outcomes.
