Support Worker needed to provide care and support for individuals with complex needs, autism, and learning disabilities.

Support adults with learning disabilities, autism, ABI, and mental health, assisting them to live fulfilling lives through various activities and personal care.

You will maintain effective communication and liaison with the Service Users, Families and others whilst respecting appropriate confidentiality. Supporting Service Users with preparation of their meals, laundry duties, cleaning and social events. Maintain accurate notes and records as and when required.

You will be responsible for ensuring outstanding care, financial sustainability, and a positive culture that supports both colleagues and the people in care. Key responsibilities include: - Leading and inspiring Service Managers and their teams to deliver high-quality, person-centred care.

You will be responsible for assessments, CPA reports, Mental Health Tribunals, Managers Hearings and will be requested to supervise junior staff members.

You will be working at Farmfield Hospital, situated located next to Gatwick.

Support adults with learning disabilities, autism, ABI, and mental health, providing tailored care and assistance with daily activities and personal needs.

You will maintain effective communication and liaison with the Service Users, Families and others whilst respecting appropriate confidentiality. You will work closely with the child's Social Worker and the Fostering service (and other services as appropriate) to ensure coordination of care and support.

You will work in Foster Carers' homes and in either our office in Woking or Reigate although you may, from time to time, be asked to assist in other areas.

You will lead the Supported Independent Living services, promote a compelling case for raising professional expectations and work collaboratively across the Council and with partners to deliver great services for residents.

You will be responsible for delivering assessments and interventions that promote independence and improve outcomes for individuals and their families or carers.
