You will be educated to HNC level in an appropriate discipline or have equivalent operational experience in a relevant professional environment.
You will have a creative, positive attitude; an awareness of data and analytics technologies and methods; and an informed, professional approach.
You will have an ability to undertake robust assessments and care planning which will enable you to update care plans to ensure the outcomes of the individuals are being met, as well as having the flexibility and skills to respond to changing needs of individuals.
You will have good organisational skills and be able to manage a caseload.
