You will hold MDT (multidisciplinary team) conversations with professionals to decide the pathway for a safe and timely discharge. Additionally, you will be able to respond to A&E and plan for care from the hospital.
You will encourage service users to take part in social activities, both within their homes and in the wider community. Building warm and trusting relationships with service users and their families is central to this role, ensuring they fully enjoy their time with us. Hours can be full-time or part-time, worked flexibly over seven days to meet the needs of the service.
