You will develop and implement comprehensive care plans and deliver targeted interventions to support individuals in managing their needs. You will be working with an outcome-focused floating support team for people over the age of 18 who are experiencing homeless, or who are at risk of homelessness and have housing related support needs.
You will manage a case-load and provide direct support to service users in accordance with their holistic support plans.
You will assist in developing essential life skills, implement care plans, and foster overall well-being, while upholding the highest standards of care and safeguarding.
You will be responsible for working with young people who are looked after and care experienced.
You will provide an assessment of needs for young people who are preparing to leave care and develop a pathway plan to meet these needs.
You will work closely with partners as corporate parents to support children in developing their life skills and working towards their aspirations.
You will also act as a Key Worker, helping individuals achieve their dreams and ambitions.
You will be paid weekly. A minimum of 12 months’ experience supporting people with learning disabilities is essential.
You must demonstrate a can-do approach, person-centred values, energy, and commitment, and be able to contribute to a positive team ethos.
You will be working on the front lines of mental health, providing stability and guidance to children who need it most. - Competitive Pay: £15.00 - £17.00 per hour (depending on experience). - Flexibility: Work patterns that respect your work-life balance. - Impact: The opportunity to witness and foster progress in children's mental health journeys.
