You will be responsible for assessing the needs of adults to determine their eligibility for care and support services.
You will promote user empowerment and independence, while also providing clear information, advice, and signposting to alternative services that can meet identified needs.
You will be required to join the care team to assist a lovely young lady who has physical and learning disabilities to support her in her fully adapted home, near Barmouth North Wales, where she lives with her Mum and siblings. The successful applicant will need to assist the young lady with all aspects of daily living and help her to engage in activities at home and in the community.
