Key Responsibilities: * Manage a caseload, providing advice, guidance, and structured care plans.
You will be a dynamic, driven individual committed to supporting residents in the Guildford and Waverley area to improve their health and wellbeing. Working closely with primary care services and other key stakeholders you will provide brief interventions, signposting and onward referral to meet individual needs.
