You will work to the thresholds set by the organization and are experienced in completing care and hospital discharge plans.
You will also need to have experience working with risk and adapting to changing risk plans. You need to be able to work within existing systems and complete discharge planning effectively.
You will work collaboratively with other professionals and agencies, such as the Police and Education Services, to meet the identified needs of children, young people, and their families. Additionally, you will complete work under the Public Law Outline (PLO) and prepare Court reports and assessments.
You must understand and actively participate in the department’s safeguarding adults’ procedures, including identification, investigation, and taking the role of Safeguarding Lead at H2.
You will work closely with other teams, including undertaking joint assessments with providers, carers, and external agencies to deliver outcome-focused services to individuals and their families.
You will coordinate the allocation of safeguarding referrals and monitor their progress.
You will also request funding for packages of care via the resource allocation group through weekly presentations and negotiations.
You will contribute to team duty rotas to provide prompt and emergency support as required.
