You will need to work at least 1 day on site per week based out of Aberystwyth.
You will be required to work with a caseload of around 20 to 25 service users / carers.
You will be expected to travel to Ceredigion and other counties when necessary to undertake assessments, reviews, care plans.
You will be part of a multi-disciplinary team that includes 4 GP Partners, 2 Salaried GPs, 3 Advanced Nurse Practitioners, 3 Practice Nurses and HCSW/Phlebotomists as well as experienced and well-established reception and administrative teams.
