You will work in a multi-disciplinary environment, collaborating with health, housing, and voluntary organisations to deliver person-centred and strength-based support to Bristol’s diverse communities. Key Responsibilities - Undertake Care Act assessments, carer assessments, and reviews, ensuring individuals receive the right level of support.

You will join a friendly and established team whose emphasis is on 'making safeguarding personal' and delivering a proportionate response to concerns raised about adults at risk. The team work with adults aged 18 and above across all areas, specialising in undertaking adult safeguarding duties in line with Section 42 of the Care Act 2014.

You will be part of a close-knit Young Lives vs Cancer social work team, working with an established NHS multi-disciplinary team and services in the community.
