You will lead plans for children with disabilities and complex needs, and be responsible for the implementation, monitoring, and reviewing of plans within multi-agency meetings relating to the assessed complex needs of children, young people, and their families. You will undertake assessments and provide immediate support and intervention for children and families in need.
You will respond to new referrals, ensuring the safeguarding and wellbeing of children at risk.
You will provide assessment, care management and social work services to children and their families, arising out of the council’s duties and powers under legislation, Council and Group policy. You will undertake effective strengths-based assessments, planning, and direct work with adults and carers, identifying needs in accordance with the Care Act 2014, statutory guidance, policy, and procedures.
You will also be responsible for safeguarding duties to protect adults from abuse or neglect.
You will manage cases including Child in Need and Child Protection, conduct visits, assessments, and meetings, organize and chair meetings with professionals and families, present cases in Child Protection and CLA reviews, and handle court work and PLO.
You will work collaboratively with other professionals and agencies to meet the identified needs of children, young people, and their families, including the MASH Service, Police, and Education Services.
You will work within a fast-paced Duty Team, act on referrals from the MASH, attend to Section 47 enquiries, and complete Single Assessments.
