You will bring to the role:

You will manage a complex and diverse caseload to safeguard the welfare of individuals, families and carers.

You will assess needs and manage risk, devise and implement support plans, commission services and evaluate outcomes, working with colleagues and partners to provide direct support to individuals and their carers.

You will ensure all aspects of a patient’s care are organized and communicated across providers.

You will conduct regular evaluations of the patient’s health status and develop personalized care plans to meet the specific needs of each patient.

You will provide access to social services, mental health support, and community resources.

You will recieve a rota in advance that allows you to plan for your days off. Please indicate on your application whether you are applying for a full or part-time role.

You will then start your night time rota, which will include additional shadowing and training to ensure you are fully equipped with all the tools and knowledge you need!

You will be working with young people aged 11 to 18 who have been placed into the care system for numerous reasons with an array of disorders, behaviours and needs and therefore we require individuals who have the confidence, understanding, patience and skill set to successfully work with young people and make a positive impact on their lives.
