You will manage and support children and young people with complex care plans, including court work related to the discharge of care orders.
You will engage with children and young people effectively to participate in their care planning and conducts safeguarding investigations and assessments.
You will manage a caseload of 20-25 clients, ensuring timely and effective interventions while also participating in the duty rota to provide additional support as needed.
You must be registered with Social Work England and have significant experience working with vulnerable adults, particularly those with mental health issues and complex needs.
You will be part of a supportive and collaborative team, committed to making a significant impact in the lives of children and their families. As a Social Worker on the Children with Disabilities Team, you will screen and assess the needs of disabled children and young people.
You will undertake Children and Families Assessments, Single Assessments, and complete reports on disabled children.
You will collaborate with health services, external agencies, and community resources to deliver holistic care. Adhering to policies and the Health and Care Professionals Council’s Code of Conduct, you will ensure compliance with the Care Act (2014).
You will empower individuals to make informed choices, respond to crises, and take responsibility for your professional development.
