You will be working alongside 4 GP partners, 2 salaried GPs, a number of GP trainees and 1 Clinical Pharmacist. You will be working autonomously, responsible for the assessment of care needs and the development, implementation and evaluation of care for dialysis patients with chronic kidney disease or acute kidney injury.
