You will manage your own patient list based on 6 sessions pw and take part in our hub triaging eConsultations.
You will perform comprehensive clinical examinations and assessments, addressing both physiological and psychological needs, and developing care plans with timely referrals in accordance with practice, PCN, and local guidelines.
You will be required to work across 2 practices within the BD8 area (depending on the number of sessions contracted for) supporting our patients. All of our practices are rated good with CQC, some of which are training practices. We are a highly supportive PCN, and the practices all work closely together.
