You will be an integral member of the Lower Annandale Home Team, where you will work in partnership with colleagues from Adult Social Work, Community Nursing, Occupational Therapy, Physiotherapy, Public Health Improvement and Patient Flow.
You will also link closely with colleagues in the wider Health and Social Care Partnership and other agencies.
