You will be conducting comprehensive Care Act assessments for individuals discharged from hospital into temporary 24-hour care placements, assessing care needs, identifying appropriate services, and coordinating care and support. This role requires timely and quality assessments to support individuals in achieving their outcomes.
You will be required to work from both sites. I have a 24-hour care package required at a ratio of 2:1 during the day and 1:1 wake carer at night, and I have 8 team members at the moment.
