You will assist service users in their daily lives with personal and practical care support based on their individual care plans.
You will help service users to learn and develop their independence skills including tasks such as shopping, cleaning, personal care and accessing the community.
You will: Provide direct 1:1 and group work support to parents, children and young people. Maintain strong links with support services within the community and make onward referrals to access support for parents, children and young people. Contribute to team meetings and provide support and/or supervision to students or volunteers.
