You will work to the thresholds set by the organization and are experienced in completing care and hospital discharge plans.
You will also need to have experience working with risk and adapting to changing risk plans. You need to be able to work within existing systems and complete discharge planning effectively.
You will coordinate the allocation of safeguarding referrals and monitor their progress.
You will also request funding for packages of care via the resource allocation group through weekly presentations and negotiations.
You will contribute to team duty rotas to provide prompt and emergency support as required.
You will use a strengths-based approach, working closely with community and voluntary groups, reablement services, and commissioned providers to support individuals effectively. Strong communication, problem-solving, and decision-making skills are essential, as you will be liaising with service users, families, and professionals to ensure the best outcomes.
You will have excellent communication and organisational skills, competent knowledge of the implementation of the Care Act 2014, and experience of managing and leading on safeguarding, managing risk, and care management.
You should hold a full UK driving license and have post-qualified experience.
