You will be a qualified professional RMN and have experience of working with people with dementia and/or mental health disorders and be able to manage a caseload under the Care Programme Approach placing the person at the centre of care in a busy but rewarding role. The ability to work in partnership with carers/family members and services in the community is essential.
You will lead, motivate, and develop your team, while working closely with external professionals, families, and commissioners to deliver outstanding outcomes for the people we support.
