A Care Assistant is needed to provide in-home complex care, including support with feeding, moving, personal care, and medical equipment.
You will play a key role in supporting patients, families and carers to navigate health and social care systems, enabling safe discharges, preventing admissions and promoting independence.
You will work closely with nurses, therapists, doctors, mental health practitioners and community services to ensure holistic and joined-up care.
