You will provide a clinical review of patients and work collaboratively with an established multi-disciplinary team and senior nursing colleagues. Keeping frailty patients closer to home where appropriate is a key NHSE aim and is transforming the management of patients outside an acute hospital setting.
You will be a qualified professional RMN and have experience of working with people with dementia and/or mental health disorders and be able to manage a caseload under the Care Programme Approach placing the person at the centre of care in a busy but rewarding role. The ability to work in partnership with carers/family members and services in the community is essential.
You will work closely with the Band 7 Team Lead to shape the critical care Occupational Therapy team and with the wider MDT (Dietitians, Physiotherapists, Psychologists, Speech and Language Therapists, Critical Care Nurses and Medical workforce). This is a part-time post of 22.5 hours per week, with pattern to be discussed after appointment.
