You will play a key role in promoting person-centred, strength-based practice that empowers residents to maintain independence.

You will also contribute to service development, quality assurance, and audit activity, ensuring continuous improvement in practice and outcomes for individuals and their families.

You will manage a caseload, provide structured 1:1 and group interventions, deliver care plans, and ensure continuity of care through close collaboration with partner agencies. This is a rewarding opportunity to make a meaningful difference in people’s lives while contributing to key outcomes for service users, commissioners, and the wider community.

You will support individuals in maintaining mental, physical, and emotional well-being and empower them to be happy, healthy and successful in achieving their life goals, working alongside their circle of support.
