You will be seeing patients who have been triaged from across our 4 PCN practices at a centralised location. These patients will have one issue requiring same day assessment and management. Complex patients e.g. pregnant patients or those on palliative care register would not be eligible to be seen in the acute hub.

You will need to hold either a Social Work or Nurse qualification and be able to show your registration with SWE/NMC is in date.

You will need to hold a Nursing or Social Work qualification (DIPSW, CQSW, CETSW, CSS or BA/Bsc / Social Work).

You must also hold a current registration with the NMC or Social Work England.
